The Eye and The Hand, lieutenants of the Cult of Vecna, former humans transformed by spells (Ken Frank, WGA4: Vecna Lives!, TSR, 1990) Most of this adventure revolves around the party uncovering and battling various agents of the cult. The Eye could work as-is for a creepy encounter, but The Hand is rather silly.
